Prospective Buyer Questionnaire

 

The purpose of this questionnaire is to insure that each & every puppy or adult placed goes to the appropriate home best suited for that individual dog therefore necessary screening must be made; it is important there be a "match" between both the Collie & the new owner and this questionnaire is beneficial in helping to assure there is a "suited match".  It also makes it easier for me to have the questionnaire to refer back to when I have a puppy or adult available.

1.    Please provide your name, mailing address, phone number(s), & email address.

 

2.    How did you find out about Jag-View Collies?

 

3.    Are you looking for a puppy or an adult?

 

4.    Are you interested in a Rough or a Smooth?

 

5.    What color are you interested in?

 

6.    Do you have a preference regarding whether you acquire a male or a female?  If you have a preference, please explain why.

 

7.    Would you consider a puppy/adult of another variety, color, or sex if your preference is not available?

 

8.    If you are interested in a Rough Collie, are you aware that they require a considerable amount of grooming?  If so, are you willing to provide the dog with this necessary grooming or would you be willing to take the dog to a groomer on a regular basis?

 

9.    If I have a Collie available for your consideration, what type of living environment will the dog be provided?

 

10.    If this dog is to spend most of it's time as an inside dog, do you plan to use a crate?  If so, how many hours will the dog have to spend in a crate at one time?

 

11.    What are your expectations and goals for this dog?  Are you interested in competition in performance events such as obedience, agility, or herding?

 

12.    Do you have any intention of breeding the dog?

 

13.    Are you married?

 

14.    Do you have children?  If so, please list their ages & genders?

 

15.    Who will be the primary caregiver of the dog?

 

16.    How long have you lived at your current residence?  Do you own or rent?  If you rent, do you have your landlord's permission to keep a dog?

 

17.    How many people reside at your home?  If other people reside at your home, do all of them want a new dog?

 

18.    Is anyone in your household allergic to dogs?  If so, how do you plan to handle the allergy problem with your new Collie?

 

19.    Have you ever owned a Collie before?  If so, what happened to the dog?

 

20.    Do you currently own any dogs?  If so, how long have you owned the dog(s), what are their ages, sexes, & breeds?

 

21.    Do you currently have any pets other than dogs?  If so, what type do you have & how many?

 

22.    If you currently have animals, who is your veterinarian & what is the name of their clinic?

 

23.    Have you ever returned a dog to a breeder?  If so, please explain why.  If you have done this more than once, please provide information about each occasion.

 

24.    Have you ever sold, traded, or given away any dog?  If so, have you ever sold, traded, or given away any dog to any pet shop, dealer or individual for the purpose of resale, research laboratory, rescue organization, animal shelter, or similar facility?  If yes to either of these questions, please explain what the circumstances were.

 

25.    Are you aware that purchasing a dog is a 12-14 year commitment, sometimes longer?  Are you committed to providing the dog with all necessary needs that the dog may have for it's entire lifetime (including but not limited to food, medical attention, grooming, shelter, & heartworm preventative)?

 

26.    If for some reason you were unable to keep the dog for it's entire life, what would you do?

 

27.    Do you have three references that I could contact?  Please provide names, phone numbers, & email addresses for those that have email.

28.    Who do you know in the Collie world that could give you a reference?

 

29.    Who do you know that shows dogs and would be available to assist you in the ring as well as in a breeding program?

 

30.    How long have you been showing?  Have you ever shown a dog to it's championship?  If so, please provide the dog's registered name & breed; if you have finished multiple champions list the names/breeds of 3-5 that you have finished.

 

31.    Are you a member of any dog clubs?  If so, please list the clubs in which you are a member.
